Patient Information

At Pain Specialists of America, we are committed to making your visits with us as convenient as possible. The information below will tell you more about referral and appointment policies.

Physician Referrals & Reports

Referrals are accepted from physicians and professionals of all specialties, with patient progress monitored closely with the referring service.

Referring physicians are sent a written report. In addition, a follow-up letter indicating treatment and progress will be sent at the time of discharge. Communication with the referring physician and the primary care physician is a high priority.

Appointment Policies

We recommend arriving early to allow enough time to register. If you need to cancel a procedure, please provide as much advance notice as possible - at least 24 hours - by contacting us at (855) 876-7246.

If you are more than 15 minutes late for your appointment, you may be rescheduled for a later date.

Patient Forms

Upon arrival to our office, please bring your New Patient forms, your insurance card and a list of all medications you are currently taking. It is your responsibility to bring a referral to our office if your insurance requires it and to keep that referral current for future visits.
